A Licence for the use of STANDARD MARK

LICENCE NO. CM/L-

1.  By virtue of the powers conferred on it by the BUREAU OF INDIAN STANDARES ACT, 1986 (63 of 1986), the BUREAU hereby grants to

 (Hereinafter called the licensee) this licence to use Standards Mark set out in the first column of the First Schedule hereto, upon or in respect of the article/process set out in the second column of the said Seclude which is manufactured in accordance with/conforms to the related Indian Standard referred to in the third column of the said Schedule as from time to time amended or revised.

2.  This licence carries the rights and obligations stipulated ion the Regulations made under the above mentioned Act. In pursuance of his obligations, the licensee shall pay in due manner and tine the scheduled Marking Fee set out in the Second Schedule hereto and maintain to the satisfaction of the Bureau the Scheme of Testing and Inspection a copy of which is attached hereto.

1. Application to be submitted through authorized person of Indian Branch of the foreign manufacturer.

    Reserve Bank of India permission document for opening a Branch in India.

5. Document authenticating establishment of the firm, such as Memorandum of Article in case Applicant Firm is a Limited Company or Partnership Deep in case the applicant firm is under Partnership.

6. Process Flow Chart showing the Manufacturing process and Document supporting the quality plan, test formats etc.

7. Details of other certification (if any) on the Product/System (Document to be enclosed).

8. Details of Pre-certification (if any) on Components/Raw Materials used in Manufacturing of the product.

9.    List of manufacturing machinery in the format

10. List of Test Equipment in the format for complete testing

11. Drawing of the Product (where applicable)

12. Scheme of testing and inspection in use or any proposed to be used (Scheme Proposal to be enclosed)

13. Qualification Certificate and Experience details of Lab Incharge/ Technical/Q.C. Personnel employed (not consultants)

14. Test report from an independent laboratory indicating conformance of the product to Indian Standard and a test report of your own laboratory using test method(s) referred therein.

15.Broad Layout Plan of the Manufacturing Premises.

16. Demand Draft of Rs.1000/-in favour of Bureau of Indian Standards payable at New Delhi.

Notes

Firm may note that:

    They should submit the application duly filled in duplicate signed and scaled with firms seal in original.

    In case of shifting of manufacturing premises BIS shall be informed.

    All the above Documents are ready and enclosed with the Application.

    Ensure complete office and firm address and information regarding previous Closed/Rejected Applications and Expired/Cancelled licence including Conviction/Prosecution (if any) in application form in case any licence(s) were held under any BIS certification scheme.

    Clearly indicate product variety to be covered (type/grade/Declared rating etc., related IS).

    All Visits/Inspections to Applicant are chargeable.

    Cost of testing of sample(s) is chargeable.

   Firm will be required to submit undertaking with respect to STI, Marking fee, brand name declaration and other terms and conditions of grant of licence in prescribed proforma after recording of application.

    It may kindly be noted that in case the information submitted by your along with your application in respect of availability of testing facilities and other details as given in the application an by you vide clause 12 of Page 6 of your application.
